1. You have elected the Extended Life Treatment (ELT) voluntarily, under no inducement or offer of compensation from Roger Williams Medical Centre (the Hospital) or any of its employees.
2. You understand that, while increased lifespan is the expected outcome of the ELT, this outcome cannot be, and is not, guaranteed by the Hospital.
3. You understand that the benefits of the ELT are reduced when the treatment is begun at a later age. At your age, 40, the median expected lifespan is 370 years, and the maximum is 408 years.
4. You understand that the ELT is not reversible once it has been performed.
5. You understand that ELT provides no protection against bacteria, viruses, genetic disorders or toxic substances.
6. You understand that side effects are known to occur in a very small number of ELT cases. These include headache, reduced blood pressure, nausea, dizziness, abdominal pain and fatigue. You should not drink alcohol, use aspirin or any prescription medication for a period of 24 hours following the ELT, except at the express direction of your physician.
7. You agree to assume the risks of the ELT described above, and will hold the Hospital, its directors, officers and employees, harmless for any injuries or damages arising out of the treatment.

In general, a good introduction to the subject. But the rhetor seems to think that everything in the 19th century begins and ends with Jane Austen.

My friends who went to P.C. told me that Rhetor Fournier was hyperthermal, but he talks like he's asleep. I wish I'd misplaced it.
